Admission Tip Sheet For Hospice And Healthcare Professionals

Hospice team will call Grace House Akron (GHA) to check bed availability and obtain an application from their website.

The Hospice team completes a GHA application for residency with the patient and/or family.

The Hospice team also completes provider applications (located on website).

Grace House staff reviews both applications and if appropriate for admission reaches out to the Hospice team to coordinate admission to Grace House Akron.

The Hospice team collaborates with GHA to determine date/ time of admission – GHA accepts admissions Monday – Friday from 8:00 AM – 5:00 PM. GHA does not accept admissions over the weekend or on holidays.

The Hospice team will order all necessary DME and medications for delivery PRIOR to patient’s arrival to GHA and Hospice team will arrange transport.

Medications must be ordered from a pharmacy that delivers. GHA will not pick up medications.

Comfort Care Kits are preferred for all patients unless they already have similar medications ordered.

She wasn’t homeless and did have the support of her family and HPCs, but she couldn’t afford private care and at 35 being in a nursing home wouldn’t have allowed her to die with dignity and grace.
